Job Details:
Provides assessment, counseling, case management, and other substance use disorders treatment to meet the needs of addictions treatment clients.
Responsibilities:
- Substance Use Disorder (SUD) and Co-Occurring Disorders (COD) screening and assessments
- SUD Diagnostic Impression
- Treatment planning
- Intake and orientation of new patients
- Facilitator of group and individual meetings and family
- Reporting and documentation in patients file, record keeping
- Psycho-Education to patient
- Determines Level of Care (LOC) based on LOCADTR (Level of Care for Alcohol and Drug Treatment Referral)
- Case Management
- Crisis Intervention
Education:
High School Diploma or equivalent required
Experience:
Demonstrates evidence of working at least 2-3 years in a chemical dependency setting.
Training/License:
Current NYS License, CASAC
Other:
De-escalation training, Supporting Recovery with Medications Treatment (MAT) course, preferred or to be completed within one year of employment
