The Clinical Transformation Lead Coordinator is a member of the Quality and Care Transformation team that functions in support for the Bon Secours Charity Health System Medical Group (BSMG) in the management of preventative and chronic disease using the principles of the patient centered medical home. One of the primary tenants of this role will be facilitating the attainment of NCQA PCMH recognition for all the primary care practices of the BSMG. A successful patient centered medical home is the cornerstone of population health strategies with regard to patient access, team-based care, managing patient populations, care management of high-risk patients, managing transitions of care and quality improvement. The Lead Coordinator is an established part of the quality incentive and value-based payment programs for the medical group The Lead Coordinator will communicate changes in the programs to the care transformation coordinators and identify opportunities for driving quality improvements. 

Responsibilities: 

  • Supervises each Clinical Transformation Coordinator in devising an approach for each individual practice to align their internal work force and create an individualized timeline for the clinical transformation project. 
  • Leads the preparation and presentation of PCMH materials, strategies, and initiatives to each of the individual practice stakeholders.
  • Supervises each Care Transformation Coordinator to maintain an established timeline for practice goals, for submission of survey. 
  • Creates, develops, and establishes the operational strategy for specialty practices of the BSMG to attain Patient Centered Specialty Practice Recognition (PCSP). 
  • Orientation leader for new employees and provide ongoing training for standardizing job specific functions and assisting in familiarizing with the Bon Secours Medical Group structure. 
  • Leads the transition and education on new NCQA and NYS standards with the Clinical Transformation Team for PCMH and PCSP Standards.
  • Coordinates with the Manager of Quality and Care Transformation, regarding tracking the status of the milestone work plan and leading team members on delivering timely materials and develops a PCMH sustainability/audit process for the preservation of PCMH recognition status, for all primary care practices of the BSMG. 
  • Navigates all metrics for the BSMG Quality Incentive Payment Programs and Value Based payment total cost of care programs. 
  • Distributes attributed patient reports to assigned clinical transformation coordination team members and help lead quality conversations with providers and practice managers. 
  • Meets regularly with the Director and Manager for the Quality and Care Transformation team to review current QIP and VBP total cost of care overall progress, key metrics, threshold targets and priority focus to maximize. 
  • Coordinates with and supports all medical practices in completion of attributed patient reviews and outreach efforts and assists with patient chart reviews to close health maintenance and care opportunities. 
  • Responsible for providing education to internal stakeholders for program specific documentation requirements and ensuring targets are achieved to maximize quality performance scores. Communicates changes in the programs to the care transformation coordinators and identify opportunities for driving quality improvements. 
  • Leads the chart review process as necessary for supplemental data submissions as part of the QIP or VBP program and when applicable upload directly into the appropriate portal for review. 
  • Performs other tasks as assigned. 

Qualifications/Requirements: 

Experience: 

3-5 years’ experience leading a primary care practice to NCQA PCMH level 3 recognition – Required. Experience with Multisite or corporate PCMH application – Preferred.

 Education: 

Bachelor’s Degree in a healthcare setting required.

 Licenses / Certifications: 

PCMH CCE required. 

Other:

 Proficient in Microsoft Office – Word, PowerPoint and Excel required. Experience with Epic EMR – Preferred. Change Management skills are essential. Excellent analytical skills for data analysis and presentations. Excellent oral and written communication skills. Exhibit sharp attention to detail and accuracy.
