Job Summary:
The Diabetes Program Manager is responsible for providing oversight for the management and operations of the Diabetes Program at Westchester Medical Center (WMC). An individual in this position participates as a member of various nursing and hospital committees, represents agency in responding to regulatory report requirements and inquiries, and works to obtain and maintain national certification for the Diabetes Program at WMC. Primary responsibilities include evaluation of Diabetes Program initiatives, staff education, clinical outcomes, scheduling, glycemic council, educational development and educational materials, and overseeing data management for this program.
Responsibilities:
Coordinates and oversees preparation and submission of diabetes education programs to meet national standards of accreditation for nurses and ancillary staff; Directs and oversees performance improvement initiatives related to diabetes and identifies trends; Participates in research activities specific to diabetes and related patient problems and diagnosis; Acts as a liaison for community support for diabetes; Plans, organizes and supervises a comprehensive Diabetes Program at WMC; Collaborates with leadership staff in the formulation and/or revision of standards of care; Acts as a resource for diabetic patients, their families, nursing and medical staff; Designs and implements a comprehensive education program for diabetic patients through use of evidence based practices/best practices; Evaluates the effectiveness and outcomes of patient education programs; Assess the educational needs of staff, and designs and implements multi-disciplinary educational programs based on current evidence and standards of care; Evaluates effectiveness and outcomes of educational programs; Collects and reports data to the leadership team; Utilizes data to identify process and program changes; Provides clinical guidance for staff in the care of diabetic patients; Provides administrative support and coordinates the management and operations of the Diabetes Program at WMC; Participates in professional programs to enhance self-development; Effectively uses automated systems to perform work assignments; Performs related duties as assigned. Qualifications/Requirements: Thorough knowledge of current professional theory, principles, methods and practice; thorough knowledge of medical principles and practices as they apply to the clinical area of specialization; knowledge of the general structure of patient-care institutions; ability to develop, implement and evaluate needs for clinical expertise in the area of specialization; ability to collect and verify valid data; ability to maintain logs and systems for data collection; ability to set priorities; ability to communicate effectively; ability to establish and maintain effective working relationships with all levels of medical, nursing, and non-professional staff, and with patients and their families; resourcefulness; flexibility; assertiveness; initiative; tact; creativity; thoroughness; sound professional judgment; physical condition commensurate with the demands of the position.
Experience:
At least three years of experience in a hospital or health care agency, one of which included development, evaluation and implementation of a diabetes program.
Education:
Bachelor’s degree, Master’s preferred and three years of experience in a hospital or health care agency, one of which included development, evaluation and implementation of a diabetes program.
Licenses / Certifications:
Possession of a valid license and current registration, issued by the New York State Education Department, as a Registered Professional Nurse or Certified Dietitian-Nutritionist.
Other:
Certified Diabetes Care and Education Specialist (CDCES) Designation preferred. Special Requirements: Education beyond the secondary level must be from an institution recognized or accredited by the Board of Regents of the New York State Education Department as a post-secondary, degree-granting institution.
