LPN/Care Coordinator State of the art Family Medicine practice in New Windsor is seeking a mature, energetic and motivated LPN to fulfill the job of Care Coordinator. The practice utilizes a completely computerized practice management system and an electronic medical record thus computer skills as well as clinical skills are required. This position works in collaboration with physicians and clinical staff to support patients with chronic conditions and/or complex needs according to guidelines set forth by practice. The Care Coordinator facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management. In addition, the LPN will provide clinical support to our three provider practice from time to time, helping to cover when other clinical staff members are on vacation or out of the office. The candidate must have at least 2 years of outpatient licensed practice nurse experience and excellent customer service skills. Bilingual skills (English/Spanish) are a plus. There is a competitive salary as well as an excellent benefits package. Care Coordinator Duties: 1. Assuring every patient within the practice has been assigned a risk stratification level. 2. Developing care plans for the highest complexity patients and perform outreach to these patients between office visits on an as needed basis. 3. Contacting all patients who have been discharged from the hospital and/or ER to reconcile medications and schedule a follow-up office visit. 4. Reviewing monthly quality and utilization reports with chief physician of the practice. 5. Assisting in the completion of required reporting for New York State and the Center for Medicare Services as listed in item 5 below. 6. Perform any other duties required to maintain participation in the Comprehensive Primary Care program and recognition as a Patient Centered Medical Home. Patient Visit

Responsibilities:

1. Meeting with assigned provider to review patient schedule and anticipate needs. 2. Preparation of examination rooms appropriate to the type and level of the patient visit. 3. Preparing the patient for the visit including taking and documentation of all vitals, chief complaints for the visit, reconciling medications and allergies and assisting the patient to and within the examination room. 4. Responsibilities:

1. Administering vaccines and other injectable medications at the direction of a provider. 2. Monitoring vaccine inventory and ordering vaccines and other medical supplies as needed. 3. Notification of test results to patients/family. 4. Triaging calls and relaying messages to assigned provider. 5. Preparing and submitting specimens for laboratory pick-up. 6. Autoclaving instruments. 7. Chart preparation for the following day’s patient schedule. 8. Inventory and stocking of examination room supplies. 9. Scan, sort and attach records and reports to patient medical records. Qualification and Skills: 1. Must be a licensed in New York State. 2. A minimum of 2 years of experience in outpatient clinical practice. 3. Proficiency in electronic medical records systems and computer skills. 4. Excellent written and oral communication and customer service skills a must. 5. Possess the ability to work well independently, as well as with other co-workers and outside contacts.
