Job Details:

The Medical Records Clerk is responsible for supporting licensed practitioners in the provision of high quality medical care to all patients.This position is accountable for efficient patient records and responsible for non-clinical office functions.


  • Assists patients in completion of necessary intake documents and requisite questionnaires.
  • Checks patients ID and maintains sign out paperwork for medical record pick-ups.
  • Maintains information within the EMR system (Epic).
  • Performs customer service duties at the front desk and assists staff with patient issues as needed.
  • Scans lab reports, consultations, office notes, messages, etc. into appropriate charts on an ongoing basis.
  • Prints chart information as needed for the licensed practitioner to review.
  • Obtains records and results for new patients.
  • Gathers incoming electronic faxes and routes incoming medical records to the appropriate chart and inbox.
  • Obtains authorizations from insurance carriers for medications and procedures as needed.
  • Forwards clinical information records to other physician offices or other parties as per protocol.
  • Handles high volume of calls and voicemails for all medical record needs or questions.
  • Works closely with the hospitals, other offices and insurance companies to ensure all medical record needs are met.
  • Performs all functions necessary for the successful implementation of the NYS immunization transmissions.
  • Follows all the HIPPA rules for giving and receiving patient information.
  • Performs other duties as assigned.

    1-3 years medical office experience and comprehensive knowledge/training of HIPAA Laws.


    High School Graduate; College Degree preferred.
