Job Details:
To render intensive casework, counseling and referral services to all dialysis and pre-dialysis patients and their families. To function as a member of a multi-disciplinary health care team to assist with the transitional and on-going needs of the dialysis patient population.
Education:
- Minimum – masters degree; preferred – certification in social work
Experience:
- Minimum – 2 yrs experience; preferred – 4 yrs experience, 1 yr. Hospital experience
Training/License:
- NYS department of education license of social service
